Scheduling an Appointment
In order to schedule an appointment with Dr. Acunzo, please
contact the office via email or phone to determine appropriateness
of referral. If after speaking to Dr. Acunzo or her staff
and an appointment is recommended, you will be directed to
complete the intake form, print & sign the release authorization
forms, and either the child or adult history forms for the patient:
Prior to your appointment, we require:
- A completed Intake Form and the Release Forms
- A completed Childhood or Adult History Form
- A deposit of $100 toward the co-payment, co-insurance,
deductible, and/or academic achievement testing.
Mail the completed forms and the deposit to Dr.
Acunzo in order to reserve your appointment.
At present, Dr. Acunzo accepts many insurances for
coverage; most require pre-authorization. Kindly note that
a pre-authorization is
not a guarantee of payment
by your health insurance provider, so please check with your
provider about requirements, deductibles and out-of-pocket
expenses. As a rule of thumb, insurances only cover expenses
that are "medically neccessary"; they do not pay for
the academic portions of Neuropsychological Testing. Clients
can expect out of pocket expenses for each evaluation.
At your appointment,
we expect to have the following:
Typically, individuals are tested for approximately 3 to
6 hours, and often in one to three sessions. School meetings
are often not covered by insurance and charged separate of
the academic test charges. The
fee for such meetings will be required prior to the meeting.
